IMPORTANT NOTICE: By submitting this form, you agree to the terms and conditions stated above. Requests for cancellation MUST be sent in writing and by mail to qualify for a refund. A 50% refund will be granted to authorized requests received ON OR BEFORE March 16th, 2025. Cancellation refund requests received AFTER March 16th, 2025 will NOT be accepted.
